Event Registration Acknowledgement and Consent *
Please read each statement below carefully. By checking each box and submitting the form, you acknowledge and agree to each specific provision as an attendee of the event hosted by JETSET Pilates.
I understand and accept that all conference tickets or sponsorships are final and non-refundable.
I acknowledge that full payment must be received by December 22, 2025, or my ticket may be canceled.
I understand that I am responsible for canceling any hotel reservation directly with the hotel, and that refunds (if any) are subject to the hotel's cancellation policy.
I understand that photography and videography may take place during the event. I hereby grant JETSET Pilates, its representatives, and affiliated partners the irrevocable, worldwide, royalty-free right and license to use, reproduce, edit, distribute, and publicly display my name, image, likeness, and voice captured at the event for promotional, marketing, advertising, and commercial purposes, in any format or media now known or later developed.
I understand that attending a live event, including travel, fitness sessions, social gatherings, and other activities, involves some personal risk. By choosing to participate, I take full responsibility for my well-being. I release JETSET Pilates, its team, and event partners from any claims related to injury, illness, or property damage that may occur during or as a result of my attendance.
